1. Please complete the Brandeis Facility waiver online.

Facility Waiver

This is what you need to fill out:

Date Attending: Nov 22 2024 (Just that first day is fine.)

School/Organization: Folk Dance Federation of California, South

Then scroll all the way to the bottom to add your Name and Signature

2. Download Registration Form (and 2 Federatiion waivers if necessary).

3. Mail completed form along with a check made out to the Folk Dance Federation of Calif. South.
